Opioid Settlement Funds Application

PLEASE NOTE that it is recommended to review the application prior to completing any of the fields. 

You will be uploading your organization’s documents, a Budget and a Project Work Plan, which are linked below. Therefore, it is recommended that you have all the required documents and complete all additional forms before completing the application.  You will not be able to save your application and return to it later. Therefore, do not navigate away from the page or you will lose the information you have already entered.
